Use data from additional CSV columns

Hello,
I have a single repository of various files whose names are all ID numbers. I have a CSV file that has:
Column A = ID numbers (file names)
Column B = new name I would like to change it to
Column C = folder name
Column D = sub-folder name

I would like to rename each file to the names in column B and have them deposited into a sub-folder (Column D) which are inside folders (Column C).

Is this possible?
