Populating many locations from list

I'm looking to use a text list to move or copy files from one path to many based on information in a CSV. The <csv> tag and the import list/data functions all seem to get me almost there, but cannot seem to get the destination path to stick.

For example, I'd like to move/copy the files from column 1 to the locations in column 2, appending a suffix or otherwise adjusting the name during the process.
